Abstract. Epilepsy is a chronic and often disabling condition with high prevalence and significant clinical heterogeneity, posing a considerable medical and social burden. Drug resistance is relatively common and may require surgical treatment in selected cases. In addition, psychiatric comorbidity is frequently observed, further complicating clinical management. We present a patient with longstanding left temporal lobe epilepsy who underwent anterior temporal lobectomy with amygdalo-hippocampectomy, resulting in complete seizure control. Despite this favorable outcome, the patient subsequently developed an organic mental disorder with anxious-depressive symptoms, preceded by a series of stressful life events. This case highlights the complex interaction between neurological vulnerability and psychosocial stressors, and underlines the importance of a multidisciplinary approach in the diagnosis and treatment of such patients.
